If you have Arc 9.3 or greater, the USNG is incorporated into the Grid styles already.  There is no need to install the extension.  As a matter of fact, it shouldnt let you install the extension.

To add the grid, use the same graticules/grids methods that you have used in the past, and just select the USNG grid style at the bottom of the Style selection list.
